The Application includes the full set of questions and measure specifications and instructions to complete the Patient Tracking Workbook.
As a reminder, only adult general acute care and specialty hospitals that care for adult inpatients and have submitted a 2023 Leapfrog Hospital Survey can access the Application via the Leapfrog Hospital Survey Dashboard and are eligible to submit the application.
As of November 8, hospitals can access the Online Application and begin entering their data. Applications are due on January 15, 2024.
