How do I review my hospital's Safety Grade on the Grade Review Website?

Leapfrog holds a courtesy Safety Grade Review Period for hospitals via the Safety Grade Review Website and a Letter Grade Embargo Period prior to the release of the Hospital Safety Grade. By accessing the Safety Grade Review Website, hospitals will be able to:

1. Review the source data used to calculate the hospital’s numerical score.
2. Download an updated copy of the Hospital Safety Grade calculator and the Scoring Methodology.
3. Preview the hospital’s numerical score and confidential Hospital Safety Grade.

Hospitals that are eligible to receive a Hospital Safety Grade will receive a username and password on the date that the courtesy Safety Grade Review Period opens.  The username and password are sent either via email or mail, depending on whether or not the hospital submitted a Leapfrog Hospital Survey by the data snapshot date.
