How do I submit a question to the Leapfrog Help Desk? What are the hours of operation?

If you have any questions regarding the Leapfrog Hospital Survey, the Leapfrog ASC Survey, the Hospital Safety Grade, or need general assistance, Leapfrog offers an electronic Help Desk. The Help Desk is staffed from 9:00 a.m. to 5:00 p.m. ET on all regular business days.

Click here to submit your question to the Leapfrog Help Desk, or click "Submit a request" at the top right of the Leapfrog Help Desk homepage.

To attach a file (e.g. Security Code Request Form) to your request, click on Add file in the Attachments field below the Question field:

The Help Desk is not staffed on federal holidays. Help Desk tickets are responded to within 1-2 business days. Hospitals should plan ahead to ensure they have a security code and are able to access the Survey well in advance of any Survey deadlines.
