How do I get a Security Code for the ASC Public Reporting Program and Survey 2.0?

Each ambulatory surgery center will need a 16-digit security code to log in to the ASC Dashboard and submit the Accreditation Certificate and the Leapfrog ASC Survey 2.0.

In order to ensure that only authorized individuals have access to the  ASC Dashboard for each facility, a Security Code Request Form must be completed.
