Leapfrog Survey experts are available for free, one-on-one Leapfrog Hospital Survey and Leapfrog ASC Public Reporting Program Orientations. These orientations are designed for individuals who are new to the Leapfrog Hospital Survey or Leapfrog ASC Public Reporting Program and would like more information on our programs and how to get started.
Orientation calls can be requested by selecting “Submit a Request” in the upper right of this page.
You will first need to select the Ratings Program (“Leapfrog Hospital Survey“ or "ASC Public Reporting”) and then complete the additional requested information. For issue, select “Hospital Survey Orientation Call” or “Orientation Call.” A member of Leapfrog’s Ratings Team will review your request and follow up with dates and times when we are available to schedule an orientation.
